How Do I Become a Member?


Eligibility
To be eligible for Family Care, you must:

• have long-term care needs

• meet financial eligibility requirements

• be a senior 65 or an adult 18 and over with a physical or developmental disability

• live in Ashland, Bayfield, Barron, Burnett, Douglas, Iron, Polk, Price, Rusk, Sawyer, or Washburn County.

How to Apply

1) Contact your county ADRC. They will visit you to do a “functional screen.” This will show what level of long-term care you may need. You will discuss the options available to you, whether it is Family Care or other services.

2) If you are interested in Family Care or another Medicaid program, the ADRC will help you contact the county economic support unit to determine your financial eligibility. (Medicaid-eligible people automatically meet the fiancial eligibilty for Family Care.)

3) If you qualify for Family Care, an enrollment consultant will call or visit in person to make sure you understand all your choices for long-term care. If you decide on Family Care, the ADRC will enroll you.

4) A NorthernBridges staff member will call to schedule your first appointment.

Other Long-Term Care Choices

People eligible for Family Care may choose to have NorthernBridges provide/coordinate their services or to fully self-direct their own services within IRIS (Include, Respect, I Self-Direct).
