About Us
NorthernBridges is a long-term care organization bringing Family Care services to 11 counties in northwest Wisconsin. Within 3 years we will serve over 3,000 members and employ 200 people.
NorthernBridges/Family Care:
• allows eligible seniors and adults with physical or developmental disabilities to live at home or in other community care settings
• eliminates waiting lists for community-based long-term care programs
• manages the increasing cost of Medicaid
But that is only part of what NorthernBridges is about...
We are really about:
• What any one of us would want for our mother or father
• Our loved ones living independently with dignity
• People staying in homes where they raised a family
• A daughter living her life knowing her parents are being cared for

Meeting Members’ Goals
NorthernBridges does this by listening to and learning about a member’s desires for their life. We call these “personal outcomes” or what is important to members and often things they wish were different in their lives.
Our mission is to “provide the best life possible” to our members by helping them achieve their personal outcomes in a cost-effective way, at home or in their community, among families and friends.
NorthernBridges serves Ashland, Bayfield, Barron, Burnett, Douglas, Iron, Polk, Price, Rusk, Sawyer, and Washburn county residents. To find out if they are eligible to become members of NorthernBridges, residents must first contact their county Aging and Disability Resource Center (ADRC).
