About Family Care
The Family Care program helps seniors and adults with disabilities live as independently as possible in their own homes or other community care settings.
Member-Centered Care Team
In Family Care, members are the most important part of a care team that works together to decide:
• What kind of life the member wants to live
• A plan to achieve that kind of life
• The supports a member needs and wants to reach those goals
Team members include:
1) the member
2 a registered nurse
3) a social services care manager
Members may also have family, friends, or anyone else they choose as part of their team.
Member Outcomes
Together the team makes the best possible choices to meet the member’s personal “outcomes.”
Examples of outcomes are:
• I decide where and with who I live.
• I make decisions regarding my support and services.
• I decide how to spend my day.
• I have relationships with family and friends I care about.
• I do things that are important to me.
• I am involved in my community.
• My life is stable.
• I am respected and treated fairly.
• I have privacy.
• I have the best possible health.
• I feel safe.
• I am free from abuse and neglect.
Provider Network
Members choose from NorthernBridges’ local provider network for needed services, such as in-home personal care, medical equipment, assisted living, transportation and help with employment.
Services from doctors, clinics, hospitals and pharmacies are paid through Medicare and/or Medicaid and not part of the Family Care program. However, the NorthernBridges’ care team will help coordinate these healthcare services with members and their doctors.
