Under the direction
of the Lead Regional Manager, the Rehabilitation Specialist provides
information and guidance to the teams regarding rehabilitation services and
durable medical equipment for members. All of this is done to support the
organization’s dedication to providing accessible, cost-effective,
high-quality, person-centered care that puts the member first. They will
perform these duties personally or through the work of their staff.

Objectives, and Responsibilities
Goal & Objective: Ensures that care is delivered in a manner that demonstrates
the Family Care program values of access, choice, quality, and
cost-effectiveness.
Related Responsibilities
Consults with teams on member needs for rehabilitation services, home
modifications and durable medical equipment (DME) needs.
Participates in service authorization discussions with teams for rehab
services, home modifications and DME services.
Completes evaluation of member needs related to rehabilitation therapy,
DME, and/or home modifications/home safety reviews as requested by the team.
Assists teams in developing and revising the service plan as needed.
Assists teams as necessary in obtaining appropriate DME from contracted
DME vendors.
Monitors rehabilitation therapy and use of DME provided to assist members
in meeting desired outcomes as requested by team.
Provides information on community resources which may assist in meeting
member outcomes as identified by member and team.
Assists with discharge planning for members needing rehabilitation,
home modifications and/or DME services.
Partners with the team to set service delivery priorities.
Assists Care Managers to develop and implement strategies for
maintaining members in non-institutional settings.
Responds to member advocacy needs.
Assists
provider network staff in identifying and evaluating vendors for rehab
services, home modifications and DME.

Related Responsibilities
Assists
management in developing policies and procedures, network development and
current service delivery and practice standards for rehabilitation services.
Meets
regularly with Care Management Supervisors and/or Care Management Regional
Managers, and Quality staff to monitor utilization of rehabilitation and DME
services and identify performance improvement initiatives.
Works
with service providers to assure quality and continuity of services.
Stays
abreast of new developments in the field of rehabilitations therapy.
Is
accessible to team members for consults on member issues, emergencies.

member excellence.
Related Responsibilities
Assists
with training new care managers on aspects related to rehabilitation therapies,
DME, and home modification needs.
Identifies
educational needs of teams and assists in planning and implementing trainings.
Performs
own functions and other duties necessary to fulfill the responsibilities of
this role. Fills- in wherever needed in order to achieve the mission and goals
of the organization or at the request of the Lead Regional Manager..
Commits
to ongoing skill-based training and mentoring opportunities.

and Experience
NorthernBridges

requirements, which provides the necessary abilities to perform the duties of
this position.
NorthernBridges
Rehabilitation Specialist must have a bachelor’s degree in Occupational or
Physical Therapy and current Wisconsin professional licensure. A minimum of three years of experience
working with individuals who have long-term care needs is preferred.
There is a strong
preference for candidates with experience in case management.

services and DME products designed to help people with long-term care needs.

Medicaid
coverage for rehabilitation services, DME and disposable medical supplies
(DMS).

concepts, theories, and practices of health and human services to long-term
care populations

The Rehabilitation Specialist
may be required to travel to other locations, and therefore will be required to
hold a valid driver’s license and have access to reliable transportation.

environment where the noise level is usually quiet. Hours of work will
generally be during regular business hours and average at least 40-45 hours a
week. There will be some variation in
work hours due to special projects, deadlines, and other concerns. Occasional out-of-town and overnight business
travel is required for business and education, by air and auto. This position
is in the No Risk Exposure category for blood borne pathogens and will follow
all recommended training and procedures for this category.
