Position Background
As

Under
the direction of the Quality Manager, the Quality Program Specialist is
responsible for assisting in the administration of an integrated quality
management system by coordinating ongoing discovery, remediation and
improvement activities. All of this is
done to support the organization’s dedication to providing accessible,
cost-effective, high-quality, person-centered care that puts the member first.
They will perform these duties personally or by collaborating with others.

member wellness, monitor adverse and/or incidents which affect member safety
and health.
Related Responsibilities
·
Participates in the development and
implementation of NorthernBridges’ prevention and wellness program
including: best practice guidelines,
best practices, coordination of and creation of articles for the Member Newsletter,
and coordination of the creation and distribution of brochures regarding
prevention and wellness.
·
Receive, process and track incidents
(adverse events and critical incidents) reported by providers and IDT
staff. Identify and monitor trends by
member, type, and/or provider.
Goal & Objective: Ensures
compliance with regulatory agencies’ and organizational quality requirements.
Related Responsibilities
·
Assist in the development and
implementation of our Quality Management Plan.
·
Assists with the establishment, tracking
and reporting of Quality Indicators determined for the organization.
·
Coordinates and conducts Internal File
Reviews (IFR) per NorthernBridges’ Operational Guideline.
·
Identifies areas of needed individual improvement
through completion of IFR’s. Works
closely with individual care managers and their supervisors to address findings
from IFR’s.
·
Identifies areas of needed organizational
improvement through completion of IFR’s.
Conducts analysis to determine trends and/or systems issues which may be
impacting organizational quality.
Proposes and develops solutions for addressing deficiencies to the
Quality Manager and CMRC Manager.
·
Collaborate with PND to develop consistent
processes to evaluate quality services.
Works with PND to follow-up on provider quality issues. Lead and/or facilitate teams to analyze
systems to ensure processes are in place to provide on-going investigation and
remediation of provider quality issues.
·
Participates in the Annual Quality Review
(AQR) and subsequent Quality Improvement Plan as necessary.
·
Assists with the state mandated Performance
Improvement Project and any additional quality improvement projects being
undertaken by NorthernBridges to include facilitation of Quality Improvement
Project Workgroups.
·
Follows up on Quality Alerts.

Related topics, to include training of new and/or current staff regarding
Quality Department Policies and Procedures.

Attends workgroups and trainings as
requested by management staff.

effective team experiences, effectively communicating both verbally and in
writing, and ability to maintain effective relationships, especially when
difficult conversations are needed.

Participate
in supervisory and leadership training for the organization.
·

ability to communicate performance expectations for care manager roles within
the hubs.
·
Ability
to communicate accountability to care managers in meeting required timelines in
accordance with contractual guidelines.
·
Identifies
training needs of team members and works with employees.
·

the ability to build relationships with team members to create a collaborative

·
Ability
to approach conflict through collaborative problem solving.
·
Coordinates
and facilitates consistent flow of information both within the hub, with other
hubs, and other departments.

and Experience
NorthernBridges

While ensuring that statutory parameters are satisfied, the organization
is willing to consider a combination of the following education, training and
experience requirements, which provides the necessary abilities to perform the
duties of this position.
NorthernBridges
Quality Coordinator must have bachelor’s degree in healthcare administration, nursing, or public administration
or related field and a minimum
of three years of closely related experience in a health and human or
healthcare services operation, preferably serving the long-term care
population.
There is a strong preference
for Registered Nurse candidates.
Working knowledge
of current concepts, theories, and practices related to home and
community-based care for long-term care populations, healthcare delivery
systems, Family Care requirements, relations, and protocols, and Family Care
complaint and grievance procedures and protocols.

The Quality
Coordinator may be required to travel to other locations, and therefore will be
required to hold a valid driver’s license and have access to reliable
transportation.

environment where the noise level is usually quiet. Hours of work will
generally be during regular business hours and average at least 45 - 50 hours a
week. There will be some variation in

travel is required for business and education, by air and auto. This position
is in the No Risk Exposure category for blood borne pathogens and will follow
all recommended training and procedures for this category.
