Nurse Care Manager

Position Background

Under the direction of the Interdisciplinary Team Lead, the Nurse Care Manager, along with the member, social services case manager, and other significant persons in the member’s life, serves as part of an interdisciplinary team (IDT). The IDT, led by the member, is charged with developing and implementing a person-centered plan of care that outlines the support necessary for the member to achieve his or her goals. As an IDT partner, Nurse’s play an important role in supporting the member’s health and wellness goals, and ensuring that the member’s choices and preferences are at the center of the team’s work. All of this is done to support the organization’s dedication to providing accessible, cost-effective, high-quality, person-centered care that puts the member first.

Goals, Objectives, and Responsibilities

Goal & Objective: Ensure members have access to necessary health-related services.
Related Responsibilities

* Assist and support members in maintaining their eligibility and enrollment, including the use of the Long-term Care Functional Screen.

* Provide nursing services such as assessment, care planning, education, counseling and referral.

* Assist the member in accessing, understanding and participating in their medical care.

* Understand the Family Care benefit package, and be able to communicate that information to the member.

* Provide ongoing strength-based, person-centered care management.

Goal & Objective: Provide the support necessary for the member to explore the full range of service options both within and outside the Family Care benefit package.
Related Responsibilities

* Work with other people in IDTs to create timely, cost-effective care plans responsive to each member’s individual health-related needs and preferences.

* Assist member and family in identifying resources.

* Coordinate health-related services with other community professionals.

* Educate members on the importance of preventative health practices, and assist them in accessing those services.

* Understand the concept of personal outcomes and be able to successfully utilize them in care planning and everyday care management.

Goal & Objective: Monitor the effectiveness of care plans and services based on member goal achievement, clinical and functional indicators, member satisfaction and events response data.
Related Responsibilities

* Utilize member outcome achievement, clinical and functional indicators, member satisfaction and events response data to improve the quality of services.

* Based on data provided, work with the IDT as needed to improve upon existing care plans.

* Participate in quality assurance and quality improvement activities.

Goal & Objective: Monitor costs associated with member care plans, and ensure the services being provided are the most cost-effective.
Related Responsibilities

* Understand and be able to use the Resource Allocation Decision method in care planning and everyday care management. This method assists the IDT to identify options, compare costs and reach agreements with parties involved.

* Involve informal supports in the member’s care plan when appropriate.

* Performs own functions and other duties necessary to fulfill the responsibilities of this role. Fills-in where needed in order to achieve the mission and goals of the organization or at the request of the IDT Lead.

* Keeps team informed of member and provider issues and consults with the team to provide coordinated services.

* Invests time and effort in building a cohesive, well-functioning team.

* Commit to ongoing skill-based training and mentoring opportunities.

Necessary Training and Experience

The NorthernBridges expects employees to understand and to incorporate the values of our organization in their day-to-day practice. While ensuring that statutory parameters are satisfied, the organization is willing to consider a combination of the following education, training and experience requirements, which provides the necessary abilities to perform the duties of this position.

* NorthernBridges Nurse Care Manager must have a bachelor’s degree (BSN) from an accredited School of Nursing or a two-year Associate Degree in nursing (ASN) and one year nursing experience and hold a current Wisconsin Registered Nurse license.

* There is a strong preference that a candidate’s work experience be with the populations that NorthernBridges serves.

* Registered Nurses must take and pass the on-line Long-Term Care Functional Screen training within 30 days of hire.

* The Registered Nurse may be required to travel to other locations, and therefore will be required to hold a valid driver’s license and have access to reliable transportation.

Work Environment

The position normally works in an office environment where the noise level is usually quiet. There will be some variation in work hours due to special projects, deadlines, and other concerns. Occasional out-of-town and overnight business travel is required for business and education, by air and auto. This position is in the Primary Risk Exposure category for blood borne pathogens and will follow all recommended training and procedures for this category.
